Fitness/CHERP Reimbursement Programs
Community Health Education Reimbursement Program (CHERP)
Members enrolled in either the HMO or POS plan are eligible to participate
in the Community Health Education Reimbursement Program or CHERP. The CHERP allows up to $150
per calendar year per family as reimbursement for community health education classes.

To file a claim, members enrolled in either the HMO or POS plan should submit the required claim form and

Fitness Center Reimbursement or Home Fitness Equipment Reimbursement
Members enrolled in the HMO Plan are eligible to receive up to $450 each plan year for reimbursement of
membership in a gym or fitness center OR up to $200 for reimbursement toward a piece of approved exercise equipment.
Gym or health club facility membership only applies for ONE covered employee/dependent/spouse (family memberships are
NOT eligible).
To file a claim, members enrolled in the HMO plan should submit the required claim form and required documentation
to ASIFlex for reimbursement. The deadline for submitting claims is the April 30th following
the close of the calendar year in which the expense was incurred.

Eyewear Reimbursement
Employees enrolled in the HMO plan may opt to be reimbursed up to $100 every two years per family member for prescription
eyeglasses (frames and lenses) and/or prescription contact lenses.
