Type of Coverage Who's Eligible Enrollment Window Effective Date of Coverage
Medical/Pharmacy and Dental Full-time permanent employees and some part-time employees who work a minimum of 30 hours per week. 45 days from date of hire/initial eligibility, or within 30 days of a Qualifying Life Event, or during Open Enrollment.

New hires (or newly eligible): 1st day of the month following the completion of one full month of regular employment.

Qualifying Life Event: 1st of the month following receipt of the enrollment form and supporting documentation, provided it’s received within 30 days of the event.

Open Enrollment: January 1st

Life Insurance Full-time permanent employees 30 days from date of hire/initial eligibility or during Open Enrollment.

Guaranteed Issue plans: 1st day of the month following the completion of one full month of full-time regular employment.

Open Enrollment: January 1st

Plans that require Evidence of Insurability: 1st day of the month following approval by Anthem Life.

Medical and/or Dependent Childcare Flexible Spending Account (FSA) Full-time permanent employees and some part-time employees who work a minimum of 30 hours per week. 45 days from date of hire/initial eligibility, or within 30 days of a Qualifying Life Event, or during Open Enrollment.
