Income Protection
Income Protection is designed to provide financial support to employees recovering from a non-work related injury or sickness that has them totally disabled and unable to perform the activities of their immediate job responsibilities with their employer.
Eligibility
- Full-time Executive Branch, State of NH government
employee, employed for one year represented by
*eligible groups, and is based on the criteria
that is applied for FMLA.
* Eligible Groups: Confidential; NEPBA;
NHTA-CS; SEA; Teamsters and Unrepresented Employees.
How it Works
- Employees should notify their State Agency Benefit Representative
and complete an application as soon as they believe their absence from
work may extend beyond three (3) days. Applications may be submitted up
to 30 days prior to a planned medical event i.e. scheduled surgery.
- It is the employee's responsibility to complete and submit Part II and
Part III to their Agency HR representative within fifteen (15) days of
notifying Human Resources of their medical event. Agency HR will then
forward the employee's application to Division of Personnel (DOP) for submittal.
- A Nurse Care Manager will reach out to the employee to clarify any
information that has been provided.
- Once you have been approved for the Income Protection Plan
(IPP) and have either met the waiting period or exhausted your sick leave;
whichever is later, you will continue to get paid through the normal
payroll process.
- In order for your application to continue through the review process,
the appropriate physician/medical provider will need to provide the IPP
Plan Administrator with Proof of Disability.
How to Apply
An employee or designee requesting Income Protection must complete
an application. No third party recommendations or requests will be considered.
There are three (3) parts to the Application:
- The Appointing Authority (or State Agency Benefits Representative) for
whom the requesting employee works must complete Part I.
- The employee requesting IPP must complete Part II.
- Part III must be completed by the requesting employee's physician or
medical practitioner.
- Part II and Part III must be returned completed to the Agency HR.
- An incomplete application may lead to denial of the employee's request
for income protection.
